Mum in hospital (2)
I had the mental health team and occupational therapist ring me thinking mum had been discharged when she hadn't. This added more stresses cos' it made me wonder if she'd been discharged without me being told. She was discharged with the wrong care plan & we heard nothing - I felt really let down and worry about her going in again.
I think everyone should be involved in the treatment & discharge plans - patient, clinician and carer. As a young carer it worries us if we don't know what's going on
